Parent/Guardian Request for Privately Engaged Service Provision
Parents/guardians may submit a request for privately engaged allied health service provision onsite for their child, by completing a request form.

Privately Engaged Service Provision in Schools Agreement
Prior to any service provision a Privately Engaged Service Provision in Schools Agreement must be completed and authorised by the College Prinicipal, for each clinician, and for each student for whom services are being provided.
The Agreement is between the parent/guardian, student, privately engaged service provider, and the College.
Service provision can only occur once the Agreement has been fully completed and signed by all parties and in accordance with the agreement terms.
The privately engage service provider is required to provide evidence of mandatory training, professional credentials, and certificates of insurances that are required of a provider (full details in page 5 of the Agreement).
